When cannibalizing a piece of equipment to repair another like item, the department is responsible for keeping track of the cannibalized parts.

1. Obtain Cannibalization form from the Surplus Property Office
2. Complete Cannibalization record and obtain approval from authorized representative
3. List the equipment on which the parts from the disassembled unit were used for repair
4. When residual is to be turned into the Surplus Property Office for final disposition, attach the Cannibalization Record to the Disposal Form
